I have been invited to participate in a study of hand preference in male and female professors, physicians, and attorneys. This research is being conducted by Dr. Nancy L. Segal at California State University, Fullerton, and I understand that it is a research project. I also understand that I am a valuable participant in research because my responses can reveal whether there are male-female difference in human handedness. I have been told that the time required for participation is 3-5 minutes. Even if I initially agree to take part, I do not have to complete the questions.

If I decide to participate, I will provide the information requested. I understand that the results will be sent to me when they are available. I also understand that there are no known risks associated with these questions beyond those of everyday life. No payment will be given for my participation.

I understand that my participation is strictly voluntary. I also have been told that research records will be kept confidential to the extent allowed by law. Data are filed in cabinets and kept in a locked office to which only the investigator and assistants have access. I understand that in any written reports or publications, no one will be identified, or identifiable, and only group data will be presented. I will receive newsletters summarizing results upon completion of the study. I also understand that the data will be kept in a locked room indefinitely so no one else will read it. 

My decision concerning whether or not to participate will not affect my future relations with California State University, or my current educational institution or place of business in any way. If I decide to participate, I am free to discontinue participation at any time. It is possible that I will not do certain [arts of the test battery if I prefer not to. In the event that I wish additional feedback this will also be provided (to me only), upon written request to Dr. Segal only. 

If I have further questions regarding my rights as a research participant, I may contact the PI, Dr. Nancy Segal, at nsegal@fullerton.edu, 657-278-2142. I may contact the Institutional Review Board (IRB) at CSUF, at 657-278-7719 who welcomes any questions. I consent to participate in the study titled; Handedness Among Male and Female Professors, Physicians and Attorneys. I give my consent to participation by checking the box that says AGREE. However, if I check DISAGREE, it means I will not participate.
